Dealing with Tommy John injury
Simmons told reporters Tuesday that he has a Tommy John ligament tear in his left elbow, Turron Davenport of ESPN.com reports. A ligament injury in a baseball pitcher's throwing elbow would normally lead to Tommy John surgery and a lengthy recovery timeline, but Simmons relayed Tuesday that he does not require a procedure at this time. He was seen in street clothes with a large brace on his left elbow during the Titans' 31-12 win over the Dolphins on Monday. Tennessee is on a bye in Week 5, and Simmons' goal is to continue to work with trainers to gain full extension in his left arm. The 2019 first-round pick said he expects to return for Week 6 against the Colts on Oct. 13. Simmons has logged 12 tackles (six solo), including 1.0 sacks, across three regular-season games this year.
Getting better
Simmons (elbow) is feeling much better Tuesday, after resting last week and sitting out the Titans' win over the Dolphins on Monday, Terry McCormick of TitanInsider.com reports. Simmons suffered the injury during Week 4's loss to the Packers and has remained sidelined since. The Mississippi State product told McCormick the injury was UCL-related and the recommendation was rest, not surgery, which seems to have worked thus far. With Tennesse having a Week 5 bye, Simmons will have until Sunday, Oct. 13 to continue his recovery before having the chance to play again.
Won't play Monday
Simmons (elbow) is inactive for Monday night's game against the Dolphins. Simmons, who was doubtful for the contest, will now target a possible return to action Oct. 13 against the Colts. In three games to date, Simmons has logged 12 tackles and a sack, with his absence Monday representing a hit to Tennessee's Week 4 defensive line.
Listed as doubtful vs. Miami
Simmons (elbow) did not practice Saturday and has been tagged as doubtful for Monday's game against the Dolphins, C. Isaiah Smalls II of the Miami Herald reports. Simmons injured his elbow during the Titans' Week 3 loss against the Packers. The injury has been severe enough for him to miss all three practices this week, and it appears unlikely that the 2019 first-round pick will suit up Monday. Keondre Coburn is the most likely candidate to see more snaps on the Titans' defensive line alongside starters T'Vondre Sweat and Sebastian Joseph-Day.

Simmons (elbow) remained sidelined for Friday's practice, David Furones of the South Florida Sun Sentinel reports. Coach Brian Callahan labeled Simmons as "day to day," Jim Wyatt of the Titans' official site reports. Through three games this season, Simmons has 11 tackles (six solo), including 1.0 sacks. The Titans will release their final injury report Saturday, as they face the Dolphins on Monday night.

Simmons handled his typical workload in the loss, playing 80 percent of the defensive snaps, compiling four total tackles and 1.0 sacks. However, it appears his status for next Monday's matchup against the Dolphins will remain in question until further clarity on the matter surfaces throughout the week.
Healthy for start of camp
Simmons (knee) is participating in practice at Titans camp this week, Nick Suss of The Tennesseean reports. Simmons went on injured reserve late last season with his knee issue and didn't take part in minicamp. The Titans were likely just being cautious with their star defender. Simmons is good to go for the start of training camp after missing five games last year. Turning 27 years old Sunday, Simmons has 26.5 career sacks across five campaigns.
